Middle age. Annual exams should include a dental check and, for breeds with heart risk, a careful listen to the heart. Lumps start to appear: have every new one needle-aspirated rather than watched. Look for slower rising, stiffness after rest and weight creep. This is the last cheap window to insure a healthy dog.

The size-adjusted chart instead adds a fixed number of human years per dog year. They agree that a dog is adult by two and diverge in the middle years. Neither is a medical measurement.
